ACLDs Job Coach provides on-site job coaching and advocacy for people we support in the intensive and follow-along phases of Supported Employment. In this position, you will: + Provide on-site job coaching for designated caseload + Complete and ensure accurate and timely documentation of SAP plans, case notes, monthlies, DDPs and other required documentation. + Conduct observational assessments on site in order to assist the person supported in learning the tasks of the job + Travel train on the public transportation system and/or roadways + Maintain case files for each person supported. + Prepare all documentation for ACCES and OPWDD on a monthly basis + Act as a liaison between the person supported, family and employer + Attend all staff meetings, Life Plans, conferences and annual training requirements + Provide overall assistance as assigned We require: + High School Diploma or equivalent; Associates Degree in Psychology, Sociology, Education or related field preferred. + Minimum of one year experience providing vocational or pre-vocational services to supported persons preferred + Complete (within first year of employment) twenty-four (24) hours of OPWDDs Innovations in Employment Supports training. + Clean, valid New York State drivers license Apply online at www.acld.org or text us for more information at 516 732-3979.
