As an LPN, you will work within our certified community behavioral health clinic (CCBHC) in Trenton, Mercer County NJ. The CCBHC program features a new approach to care designed to improve overall health by treating the whole person through community based mental health and addiction treatment.

Schedule: Mon-Fri 8:30a-5:00p with one shift per week 10:30a-7:00p; 30 minute lunch

Responsibilities:

  • Provides injections, reviews lab reports, contacts pharmacies on behalf of prescribers
  • Respond to patient's medication questions about side effects, usage, etc., by consulting with the prescriber as necessary
  • Complete medication profile and other documentation as required
  • Assist prescribers with various aspects of medication education and management, including groups
  • Administers injections and medications for clients who require assistance in taking medication at the program
  • Maintains inventory of medication samples, syringes and other medical supplies according to agency policy
  • Completes preliminary nursing assessment and arranges for clients to obtain physicals and other medical care as needed
  • Provides back-up to other nurses to ensure that critical services are provided to consumers
  • Assist with scheduling of appointments with Oaks prescribers as well as with specialty or primary care physicians.
  • Secure screening, preventative care, and early intervention for medical problems, including lab work; annual physicals, gynecological, eye, hearing, and dental exams
  • Engage consumers in monitoring health and wellness by maintaining a central database to track key health indicators, including blood pressure, blood glucose (or HbA1C), lipid profile, weight, waist circumference, BMI, etc.
  • Coordinate with treating physicians and nurses regarding medication, follow-up treatment, sharing of records, and implementing behavioral medicine and evidence-based illness management protocols
  • Accompany consumers on appointments
  • Provide disease management and illness management services, including evidence-based motivational interventions to increase treatment compliance, support lifestyle changes, and manage the co-morbid anxiety/depression that often accompany chronic diseases
  • Provide assistance to consumers in safely taking multiple medications for both physical and mental health disorders, avoiding polypharmacy, and managing multiple medical concerns
  • Provide education, individually and via facilitating or co-facilitating groups, and support for healthy lifestyle changes, including reduced dependence on tobacco, alcohol, and other harmful substances, improved nutrition
  • Participates in team meetings and provides consultation to other staff re: medical issues which may impact on mental health and best practices re: health and wellness

      Qualifications:

      • Licensed Practical Nurse (LPN) currently licensed in NJ;
      • Experience working in mental health field preferred;
      • Valid Driver's License required.
        Vaccination required per EO 283
