$70-$90-hr
We are looking for a motivated Physician Assistant or Nurse Practitioner to join our medical practice in Levittown, NY. We are looking for a provider who can join us providing direct patient care within 2 of our offices, either part time or full time. We have a need Monday thru Friday at our office in Levittown with weekend availability at our office in Farmingdale. Our ideal PA-NP is comfortable working within an integrated setting, collaborating with a knowledgeable and well-respected team of professionals, to offer a comprehensive approach to patients health and wellness goals. Job duties include performing trigger point injections under ultrasound guidance.

Duties:
Patient exam-evaluation (medical history, physical exam, review pertinent med records-imaging)
Diagnosis and treatment plan formulation
Patient education as to diagnosis, treatment options, rehab-exercises and various modalities
Manage patient care
Treatments- perform trigger point injections under ultrasound guidance.
Prescribe medical equipment as needed
Documentation and progress notes- EMR
Collaboration with the multidisciplinary team for comprehensive patient care

Requirements:
Graduation from an accredited Physician Assistant or Nurse Practitioner program
PA-NP license in NY
Experience with trigger point injections with ultrasound guidance a plus

Compensation:
$70-$90 per hour

Schedule:
Monday through Friday 1:30pm to 7:30pm
Weekend availability (Sat 9:30- 3:30)
Flexible Schedule
Full time or Part time Availabilty
