Job Description:

We are seeking a compassionate and experienced Field Registered Nurse (RN) Certified Home Health Aide (CHHA) specializing in pediatric care to join our client's team. In this role, you will provide essential medical care and support to pediatric patients in the comfort of their homes, ensuring they receive the specialized attention and assistance they need for optimal health and development.*Key Responsibilities:** Conduct comprehensive assessments of pediatric patients, considering their unique developmental stages, medical conditions, and psychosocial needs.* Collaborate with pediatricians, specialists, and interdisciplinary team members to develop individualized care plans that address the specific needs and goals of each child and family.* Continuously evaluate patients' progress and adjust care plans as necessary to promote growth, development, and overall well-being.* Provide skilled nursing care to pediatric patients, including medication administration, monitoring of vital signs, and management of acute and chronic conditions.* Administer treatments, therapies, and interventions as prescribed by healthcare providers, ensuring safe and effective delivery of care.* Educate parents and caregivers on proper medication administration techniques, potential side effects, and signs of complications to watch for.* Support parents and caregivers in understanding their child's medical needs, treatment plans, and developmental milestones, fostering a collaborative approach to care.* Provide education and guidance on pediatric health and wellness topics, including nutrition, immunizations, and injury prevention strategies.* Offer emotional support and counseling to families coping with the challenges of caring for a child with complex medical needs, promoting resilience and coping skills.* Assist pediatric patients with ADLs, such as feeding, bathing, dressing, and mobility, while promoting independence and self-care skills appropriate to their developmental level.* Collaborate with occupational therapists, physical therapists, and speech-language pathologists to implement personalized interventions to enhance functional abilities and promote optimal growth and development.* Advocate for the needs and rights of pediatric patients and their families, ensuring they receive the support and resources necessary to thrive in their home environment.* Maintain accurate and detailed documentation of all patient assessments, interventions, and outcomes, adhering to regulatory standards and agency policies.* Communicate effectively with healthcare team members, including physicians, therapists, and social workers, to coordinate care and ensure continuity of services.* Participate in interdisciplinary meetings and case conferences to review patient progress, discuss care plans, and address any concerns or challenges.*Requirements:** Current RN license in the state of practice.* Certification as a Home Health Aide (CHHA) preferred.* Minimum of 6 months of clinical experience in nursing.* Strong assessment, clinical judgment, and pediatric-specific nursing skills.* Excellent communication and interpersonal skills, with the ability to engage children and families in a compassionate and supportive manner._Please note- The annual base salary range is $90k to $110k. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things.
