Client RN License required Certifications: BLS (AHA) required, PRI
certification to be obtained within 3 months of employment, Screen
certification preferred, Care Management certification preferred
(e.g., CMC, CCM, etc.) Must-Have: - Discharge experience required -
Knowledge of healthcare financial, regulatory, and payer issues
preferred - Knowledge of state, local, and federal programs
strongly preferred - Experience in the use of Milliman/InterQual
criteria preferred - Computer skills essential Description: The RN
Care Manager performs comprehensive assessments on admission in
accordance with Care Management policy. The RN screens all patients
using established tools for high-risk indicators to ensure
appropriate supportive services for discharge. The RN assesses all
populations for potential discharge planning needs. The RN provides
ongoing reassessment of needs throughout the hospital course. The
RN performs Utilization Review (UR) activities per department
policy. The RN understands and adheres to performance standards,
policies, and behaviors. The RN completes a comprehensive initial
assessment. The RN performs multidisciplinary care coordination at
intra-hospital and inter-hospital levels. The RN performs all
transitional planning activities, including patient and caregiver
education, arrangement of aftercare, and involvement of commercial
payers. The RN communicates and collaborates relevant patient
information with appropriate health team members. The RN ensures
departmental goals of length of stay reduction, readmission
prevention, denial reduction, and satisfaction improvement are
achieved. The RN documents per department requirements. The RN
participates in performance improvement activities as needed. The
RN performs all UR activities according to department policy,
including commercial payer reviews and admission clinical review.
The RN collaborates with all healthcare professionals to evaluate
the discharge needs of geriatric patients. The RN performs all
other related duties as assigned.
