Job Description Job Description Regional Field Registered Nurse (RN) – Home Visits Location: Bronx, NY Schedule: Monday–Friday | 8:30 AM – 5:30 PM (1-hour lunch) Field-Based: 100% in the community Employment Type: Contract | Temp-to-Perm Potential Openings: 2 We are seeking experienced and reliable Registered Nurses (RNs) to provide field-based clinical care through routine home visits across the Bronx. This role is ideal for nurses with a home care or hospital background who enjoy autonomy, hands-on patient care, and consistent weekday scheduling with limited weekend rotation. Must-Have Qualifications Active New York State Registered Nurse (RN) license Home care experience (CHHA experience strongly preferred) Ability to work 100% in the field and complete daily home visits Valid driver’s license and reliable personal vehicle Strong clinical assessment and documentation skills Ability to manage an average caseload of ~25 visits per week Comfortable performing in-home care independently Availability to work 1–2 rotating weekends per month Nice-to-Have Qualifications Hospital RN experience Wound care experience (any level) Experience administering medications in the home setting Familiarity with community-based or managed care programs UAS experience (training can be provided if needed) Key Responsibilities Conduct routine in-home nursing visits across the Bronx Perform wound care, medication administration, and general health assessments Monitor patient conditions and report changes promptly Coordinate care with physicians and interdisciplinary teams Ensure accurate and timely clinical documentation Manage hospital follow-ups and continuity of care as needed Adhere to all clinical, safety, and regulatory standards Schedule & Work Structure Monday–Friday, 8:30 AM – 5:30 PM Rotating 1–2 weekends per month When scheduled for a weekend, the RN receives: One weekday off prior to the Saturday shift One weekday off following the Sunday shift Fully field-based role with structured daily visits Why This Role Stands Out Consistent weekday schedule with limited weekend rotation Autonomy and independence in field-based nursing Strong clinical focus (hands-on care, not assessment-only) Opportunity for temp-to-perm conversion Meaningful work supporting patients in their homes If you’re an RN with home care or hospital experience looking for a structured field role with long-term potential, this is an excellent opportunity.
