Job Description:

Prevoc Counselor Primary Purpose of Job Provide on-site instruction, advocacy and transportation for people we support in Prevocational Program Essential Functions + Provide on-site instruction for designated caseload + Complete and ensure accurate and timely documentation of plans, case notes, monthlies, DDPs and other required documentation. + Conduct observational assessments on site in order to assist the person in learning the tasks of the job + Travel train on the public transportation system and/or roadways + Maintain case files for each person Prepare all documentation for OPWDD on a daily and monthly basis + Attend all staff meetings, Life Plan, conferences and in-service trainings + Provide overall assistance as assigned Requirements + High School Diploma or equivalent; Associates Degree in Psychology, Sociology, Education or related field preferred. + Minimum of one year experience providing vocational or pre-vocational services to supported persons preferred + Complete (within first year of employment) twenty-four (24) hours of OPWDDs Eleversity in Employment Supports training. + Clean, valid New York State drivers license
